Parent restrictions:

If a former patient wishes to access their health record, they must provide UVA Health Information Management (HIM) with legal proof of identity, such as a driver's license or birth certificate. If the patient is deceased, the requestor must provide proof of the patient's death, such as a death certificate, along with documentation establishing that the requestor is the patient's legally authorized representative or the executor of the patient's estate.

HIM will review the request and advise Historical Collections Services whether the record may be released. If HIM authorizes the release, Historical Collections Services will mail a paper copy of the record to the approved requestor.

Parent terms of access:

A portion of this collection contains materials subject to the Health Insurance Portability and Accountability Act (HIPAA) and other applicable privacy laws. Access to these materials is restricted and may be granted only under specific conditions, including approved institutional review board (IRB) protocols, preparatory-to-research review by authorized University of Virginia Health employees, or access by individuals to their own records through Health Information Management.

Researchers must comply with all applicable privacy regulations, including requirements for de-identification prior to publication.
