If a former patient wishes to access their health record, they must provide UVA Health Information Management (HIM) with legal proof of identity, such as a driver's license or birth certificate. If the patient is deceased, the requestor must provide proof of the patient's death, such as a death certificate, along with documentation establishing that the requestor is the patient's legally authorized representative or the executor of the patient's estate.

Documentation must be submitted to Gloria Johnson and Scottie Frey using one of the following methods:

HIM will review the request and advise Historical Collections & Services whether the record may be released. If HIM authorizes the release, Historical Collections & Services will mail a paper copy of the record to the approved requestor.
