Abstract Or Scope

One "Physician's Record", a small pamphlet used by doctors or other medical professionals to document births. This record was used from 1936-1944, mainly for births in Upshur County. These books were issued by the West Virginia State Department of Health to physicians for purposes of statistics and record-keeping. Each page of the book contains a form that can be filled in with the relevant information, including the infant's name, date of birth, birthplace, race, and legitimacy, as well as the parents' names, ages, races, occupations, addresses, and previous children. At the bottom of the form is a space for the physician to date when the birth certificate had been delivered to the registrar.
